I ran into similar problems and finally figured out what was causing the issue.   All my hosts / hostname etc. settings were correct, ports available etc., but the thing that caused the heartbeat to fail.....was that NTPD service has to be running on all hosts in order to "synchronize" the heartbeat. No sync == no heartbeat.   Can't believe that did not occur to me sooner. Hope this helps someone.
